Age-dependent shift-to-the-right in the localization of colorectal adenomas.

The age-dependent prevalence and topographical distribution of colorectal adenomas was investigated in 1006 unselected autopsies (554 males and 452 females) in Mainz, FRG. In 200 out of 1006 autopsies (19.8%) a total of 498 adenomas of the large intestine were detected. The percentage of patients with adenomas increased continuously with age. Only 6% of all adenomas were localized in the caecum and 8% in the rectum, whereas all the other adenomas were distributed rather evenly throughout the ascending colon (23%), the transverse colon (25%), the descending colon (15%) and the sigmoid colon (23%). Background and Study Aims: The purpose of this study was to evaluate dark-lumen magnetic resonance (MR) colonography prospectively in patients with incomplete conventional colonoscopy. Patients and Methods: Thirty-two patients with incomplete conventional colonoscopy underwent same-day dark-lumen MR colonography on the basis of a standard protocol. Variant Arterial Supply of the Descending Colon by the Coeliac Trunk: A Case Report

Background and Objectives: Knowledge of arterial variations of the intestines is of great importance in visceral surgery and interventional radiology. Materials and Methods: An unusual variation in the blood supply of the descending colon was observed in a Caucasian female body donor. Results: In this case, the left colic artery that regularly derives from the inferior mesenteric artery supplying the descending colon was instead a branch of the common hepatic artery. Conclusions: Here, we describe the very rare case of an aberrant left colic artery arising from the common hepatic artery in a dissection study.
Intussusceptive Remodeling of Vascular Branch Angles in Chemically-Induced Murine Colitis

Intussusceptive angiogenesis is a developmental process linked to both blood vessel replication and remodeling in development. To investigate the prediction that the process of intussusceptive angiogenesis is associated with vessel angle remodeling in adult mice, we systematically evaluated corrosion casts of the mucosal plexus in mice with trinitrobenzesulphonic acid (TNBS)-induced and dextran sodium sulfate (DSS)-induced colitis. The mice demonstrated a significant decrease in vessel angles in both TNBS-induced and DSS-induced colitis within 4 weeks of the onset of colitis (p<.001). Fecal Blood Loss in Patients With Colonic Polyps: A Comparison of Measurements With 51Chromium-Labeled Erythrocytes and With the Haemoccult Test

Abstract The quantitative determinations of fecal daily blood loss after intravenous administration of 51Cr-labeled erythrocytes in 44 patients with colonic polyps and in 11 controls were compared with the results of the daily performed Haemoccult test without dietary restrictions. A total of 642 stool specimens was analyzed for 51Cr loss and the Haemoccult test. The mean fecal daily blood loss in the 34 patients with adenomatous polyps of the descending colon and rectosigmoid was 1.36 ± 0.14 ml/day (mean ± SEM), in the 10 patients with polyps of the ascending and transverse colon it was 1.28 ± 0.31 ml/day, and in the 11 controls 0.62 ± 0.07 ml/day. A case is reported with aganglionosis of the rectum, sigma, and descending colon; dysganglionosis with heterotopic ganglionic cells in the muscularis propria of the hypoganglionic transverse colon; and extreme hypoganglionosis (without detection of ganglionic cells) of the ascending colon and distal ileum. The ileum showed a transition zone with hypoganglionosis and intestinal neuronal dysplasia (IND) type B. As to the etiology of such complex intestinal innervation defects, pre- and perinatal perfusion deficits must be considered because their localization seems to be linked to the vascular anatomy of the colon. Clinical data and radiographic findings of 32 newborn suffering from necrotising enterocolitis were analysed [12 patients with rotavirus-positive necrotising enterocolitis (RV + NEC), 20 patients with rotavirus-negative necrotising enterocolitis (RV-NEC)]. In a short-circuited mucosa-submucosa preparation of the rat descending colon with preserved Meissner's plexus mounted as an everted sac rhein transiently increased the potential difference and short-circuit current (Isc) when administered serosally and mucosally, but serosal application was more effective. Maximal effects were obtained at rhein concentrations of 1.6 X 10(-4) and 3 X 10(-4) mol/l. Net chloride absorption was decreased. Indomethacin (5 X 10(-6) mol/l) did not inhibit the increase of Isc, but omission of calcium from the serosal side as well as tetrodoxin (2 X 10(-7) mol/l) decreased it by 50 and 60%. The root of the inferior mesenteric artery is an important area where autonomic nerves are close by, the dissection can be hard and laborious in order to avoid nerve damage; and the separation of mesocolon and retroperitoneum in this area can sometimes be difficult. The inferior mesenteric vein dissection is an alternative option as the first step during medial to lateral approach of laparoscopic left mesocolon mobilisation, as we have shown in this video based on two real cases and two anatomical cadaveric dissection.
